Current donors with a Donor Advised Fund or Corporate Advised Fund may submit recommendations to the Board of Trustees throughout the year. To submit recommendations for one or more grants, use the following link:Advised Fund Recommendation Form.
 
Simply use the drop-down list to locate your fund name. Then continue to complete the information using this Online writeable form. Print the form, sign it and either email, fax or mail via U.S. Post to the Foundation office. The various contact information is listed at the bottom of the form.
